This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit our website, submit a form, contact us, or otherwise interact with us in connection with our services.

Care Connect helps families, at no cost to them, identify senior living and care options that fit their needs, budget, and location. Care Connect currently serves families in New Jersey and Pennsylvania, and families do not pay Care Connect directly for its referral services.

By using our website or providing information to us, you acknowledge the practices described in this Privacy Policy.

  1. Information We Collect

We may collect information you voluntarily provide to us through our website, including when you complete a contact form or otherwise communicate with us. Depending on the form, that information may include your full name or business name, email address, phone number, message, the type of help you need, and your preferred contact method. The website does not currently offer account creation, user logins, quizzes, assessments, scheduling tools, chat widgets, downloadable resources, or public user-submitted content.

After initial contact, and only with your consent or the consent of the appropriate decision-maker, Care Connect may collect additional intake or pre-screen information in person, by phone, or through follow-up communications in order to help identify appropriate care options. That information may include care needs, diagnoses, cognitive status, behavioral concerns, activities of daily living, transfer and mobility needs, medication support, skin integrity concerns, diet needs, financial and budget information, payer sources, Medicaid status, veteran status, preferred geography, family goals, barriers, and other placement-related information.

We may also automatically collect limited website and device information, such as browser type, device information, pages visited, referral source, and general website interaction data through analytics and similar technologies.

  1. How We Use Information

We may use the information we collect to respond to your inquiry, contact you by phone, email, or text, route your inquiry to the appropriate Care Coordinator, understand your needs, identify and present appropriate senior living or care options, maintain records of communications and referrals, operate and improve our website and business processes, analyze general website performance, and comply with legal, operational, or recordkeeping obligations. Care Connect is compensated by participating providers or service partners when appropriate, but families do not pay Care Connect for its services.

  1. When We Share Information

Information submitted through the website is generally received at help@careconnectseniorliving.com and forwarded internally to the most appropriate Care Coordinator for follow-up. Care Connect does not sell personal information and does not share website inquiry information with providers merely because someone submitted a form. Information is shared with providers, communities, agencies, attorneys, realtors, or other referral partners only after Care Connect has spoken with the inquiring party or authorized decision-maker and received consent to do so.

We may also disclose information to service providers that help us operate our business systems, to professional advisors when reasonably necessary, if required by law or legal process, to protect rights, safety, or property, or in connection with a merger, restructuring, financing, or sale of assets. Care Connect has indicated that contractors or virtual assistants do not currently access lead data.

  1. Communications

If you contact us or submit information through our website, you consent to receive communications from Care Connect related to your inquiry, including by phone, email, and, where appropriate, text message. Care Connect does not currently use an autodialer, mass texting platform, or SMS automation for website inquiries, and calls are not recorded at this time.

  1. Analytics, Cookies, and Similar Technologies

Care Connect uses limited website analytics and related technologies to better understand general website traffic and performance. Care Connect has indicated that its website currently has Google Analytics, Google Tag Manager, Google Ads, and Meta Pixel installed or connected, but that Care Connect is not actively using website tracking data for advertising or retargeting based on collected personal information at this time. Website form submissions are not automatically transmitted to third parties through those forms.

Like many websites, Care Connect’s website platform or connected services may use cookies or similar technologies for analytics or operational purposes. Care Connect does not currently use a separate cookie consent banner on its website. Users may be able to manage certain cookie or tracking preferences through browser or device settings, although not all technologies can be disabled that way.

  1. Data Retention

Care Connect retains information for as long as reasonably necessary for relationship management, referral activity, recordkeeping, operational, legal, and business purposes. Care Connect has indicated that website lead and client-related information may remain in Gmail, Google Drive, HubSpot, and business devices indefinitely because formal system and device retention policies are not yet in place.

  1. Data Security

Care Connect uses reasonable administrative and technical measures to help protect information in its possession. However, no website, email transmission, internet transmission, cloud platform, or electronic storage system can be guaranteed to be completely secure. You provide information at your own risk.

Care Connect has also indicated that the website itself is not represented as HIPAA compliant and should not be treated as a secure patient portal or medical records platform.

  1. Access, Correction, and Deletion Requests

You may contact Care Connect to request access to, correction of, or deletion of personal information you have provided. Care Connect has indicated that, if such a request is received, it would search relevant systems, including Gmail, Google Drive, HubSpot, and business devices, for responsive information. Responses to such requests may be subject to legal, operational, recordkeeping, and legitimate business considerations.

  1. Third Party Websites

Care Connect does not currently maintain third party links across the general website except for sources or footnotes used in published articles. If you click through to a third party website, Care Connect is not responsible for that website’s privacy, security, content, or practices. You should review the privacy policy of any third party website you visit.

  1. Children’s Privacy

Care Connect’s website and services are intended for adults seeking information or assistance related to senior living, care, and transition planning. The website is not directed to children.

  1. Sensitive Information and Scope of Services

As part of its referral and intake process, Care Connect may receive sensitive personal information after direct contact and consent, including health-related, functional, behavioral, financial, and payer-related information relevant to identifying appropriate care options. This information is typically gathered during an in-person or phone-based intake or pre-screen process rather than through a public website form.

Care Connect’s intake and pre-screen process is intended for preliminary review and referral support only. It is not a clinical assessment and is not a guarantee of placement or admission. Final determinations remain the responsibility of the receiving provider or community.
